Services Provided: Section 28 Habilitation Services

Downeast Horizons Day Habilitation Program serves children from age 3 to 21 with cognitive impairments. Section 28 Habilitation Services take place in the child’s home or community setting.

The goal of these services is an increase in a child’s level of function, increased skill development and a decrease in maladaptive behaviors.  The utilization of the family, parent/guardian and any natural supports is critical.

Provisions of service shall conform to Chapter II, Section 28 of the MaineCare Benefits Manual (MCBM) regarding Day Habilitation Services.

Individual Habilitation Plan

An individual treatment plan is developed based on the strengths and needs of the individual.  Downeast Horizons will provide a Children’s Direct Service Professional to provide one-on-one support services in the home and community to work on specific goals and objectives outlined in the plan.  The focus is to teach necessary skills to improve the individual’s independence with activities of daily living, community integration, and behavior management.

Eligibility

Child is birth to age 21 (DEH supports children starting at age 3.)

The child/adolescent must have a cognitive disability or an autism spectrum disorder as his/her primary diagnosis and meet eligibility for service criteria specified in the MaineCare Benefits Manual Section 28.02 A. and B. below:

A. General Eligibility Criteria:  The child must be eligible for the MaineCare program.

B. Specific Eligibility Criteria:  The child must have a condition or disorder that satisfies the definition of either:

1. Mental retardation, as defined at 34B M.R.S.A. sec. 5001, or

2. Autism, as that term is defined at 34B M.R.S.A. sec.6002.
