Hourly Home Support

Hourly Home Support is direct support provided to improve and maintain a person’s ability to live as independently as possible in his or her home. Hourly Home Support is one staff to one consumer of direct support.

 

The direct service staff provide home supports/personal assistance (self-care, self-management) in order to improve and maintain a person’s health and wellbeing and his or her ability to live in and be a part of their community. Downeast Horizons’ staff also provides personal assistance with activities that contribute to a person’s health and well-being including meal preparation, safety, personal hygiene, and other activities of daily living. The type of support an individual receives depends upon the needs and as outlined in their Person Centered Plan (PCP).

The number of hours per week an individual receives are determined by that person’s needs and his or her treatment team. In most cases the Hourly Home Supports are regularly scheduled blocks of hours.

Direct service Professionals

The hourly home support staffing is structured in a manner that meets the needs of the individual(s) residing in the home. These supports are staffed by qualified, caring individuals who provide a positive, friendly non-threatening supportive care. Each individual’s care is determined by his or her treatment team. Downeast Horizons’ DSPs have access to a twenty-four hour a day, seven days a week agency on-call system.

Staff Training

DEHI staff is trained specifically to meet the needs of those supported. This training includes: Direct Support Professional Training, First Aid, CPR, Medication Administration, and may include additional training such as Mandt.

Supervisory Oversight

Each Direct Service Professional is overseen by a site manager. The site manager is a working manager who provides direct care to the residents and supportive guidance to the Direct Care Staff. DEHI believes its DSPs’ effectiveness is enhanced by increased access to their supervisor.

Community Involvement

DEHI believes in full community inclusion. DEHI staff will provide supported access to the community.

Community Support/Day Programs

Individuals who receive hourly home supports have the option to participate in a Community Support/Day Program. DEHI offers Community Support/Day Program opportunities in Bar Harbor, Ellsworth and Brewer.

Transportation

Downeast Horizons’ one-to-one hourly staff provide staff-supported transportation that is necessary for effective treatment.

All agency-sponsored activities are paid for by Downeast Horizons.
