Residential Services
Downeast Horizons operates residential support homes in Ellsworth, Hancock and Bar Harbor. Each home has two to six beds. All homes are overseen by Downeast Horizons’ management team and the Department of Health and Human Services offices of Developmental Disabilities. In addition to this oversight, our three, four, five, and six-bed homes are licensed by the Department of Health and Human Services.



Within each home, direct service staff provide home supports in order to improve and maintain a person’s health and well-being and his or her ability to live in and be a part of their community. The level of supports an individual receives depends upon an individual’s needs and as outlined in the person’s Person Centered Plan (PCP). Downeast Horizons’ staff also provide personal assistance with activities that contribute to a person’s health and well-being including meal preparation, safety, personal hygiene and other activities of daily living.
Residential Staff – Direct Service Professionals
The home environment and staffing is structured in a manner that meets the needs of the individual(s)
residing in the home. Our residential settings are staffed by qualified/caring individuals who provide a positive, friendly non-threatening supportive home environment. Each individual’s staffing level is be determined by his or her treatment team. All Downeast Horizon homes are staffed twenty-four hours a day, seven days a week. Downeast Horizons’ DSPs have access to a twenty-four hour a day, seven days a week agency on-call system.
Supervisory Oversight
Each home operated by DEHI has an assigned site manager. The site manager is a working manager who provides direct care to the residents of the home and supportive guidance to the home’s Direct Care Staff. DEHI believes its DSPs’ effectiveness is enhanced by increased access to their supervisor.
Community Involvement
DEHI believes in full community inclusion. DEHI staff will provide supported access to the community.
Community Support-Day Program
Individuals supported in residential settings have the option to participate in a Community Support – Day Program. DEHI offers Community Support-Day Program opportunities in Bar Harbor, Ellsworth and Brewer.
Community Supports activities are overseen by Direct Support Professionals who receive: College Support Direct Care Provider Training, CRMA Medication Instruction and CPR/AED & First Aid.
Transportation
DEHI provides safe reliable staff-supported transportation.
